It's a bit like low level understanding. Your already forming thoughts without words before they are translated into words. It makes it incredibly difficult to not be distracted, because anything can inject itself into our thought processes. We tend to be more conceptual and spacally driven. I think a lot of ADHD are no internal monologgers. Evolutiontionarally speaking it makes sense, we did so much less thinking, and had to be so much more alert and ready to react.
